OSHA 300 logs document several instances where employees received burns to their legs and feet due to using this spray.      -

29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed:  On or about April 7, 2017, the employer failed to ensure that their Hazardous Energy Control Procedures for Lock Out Tag Out was reviewed at least annually to ensure that all the information contained is current and up to date with what exists in their facility including but not limited to an inventory list of current equipment/machinery and any authorized personnel changes.  The last documented review was approximately May of 2015 where the employers procedure identified the need for a periodic review of the procedure.
