29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: Employees engaged in unsafe roofing operations and were exposed to serious hazards. This was most recently documented on April 11, 2017 at Lot A Tsunami Court Blue Springs, MO 64064. The employees did not recognize and avoid unsafe work conditions while working under loads without the use of a hard hat could result in a struck-by hazard. The employees also did not know the dangers of working from an elevated height without the use of fall protection. As well as the electrical shock hazards associated with working with equipment without proper strain relief. All employees had not been trained on the hazards associated with their job site.

This was most recently documented on April 11, 2017 at Lot A Tsunami Court Blue Springs, MO 64064. Employees and The Owner were observed using Skilsaws with exposed wires; the flexible cord was not connected to a device or fitting that provided strain relief or prevented pull from being directly transmitted to joints or screws.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): Employees were exposed to fall hazards when a personal fall arrest system was not provided to employees working above 6 feet in residential construction. This was most recently documented on April 11, 2017 at Lot A Tsunami Court Blue Springs, MO 64064. Employees were framing the ceiling of the first floor of a two story house. Employees were working over the leading edge of the house approximately 12 feet above the ground without the use of any form of fall protection.
