29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. On or about April 2017, the employer did not ensure that each employee was sufficiently trained to recognize and avoid all hazards associated with throwing roof materials into a dumpster from a roof that was approximately 22 feet from the ground level below without a chute.
