29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. Employees were exposed to fall hazards from unguarded skylights. Employees were exposed to a fall of at least 19 feet from the roof to the concrete floor below. Employees were not protected by personal fall arrest systems, covers or guardrail systems erected around the skylights.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: At a commercial building located at 13902 Century Lane, Grandview, Missouri: Employees were exposed to hazardous chemicals, such as but not limited to, Aldocoat 730 Skylight Coating, Puma XL Acrylic Elastomeric Top Coat, and other chemicals. Aldocoat 730 Skylight Coating and Puma XL are both harmful if swallowed, and both can cause skin and eye irritation. There was no written hazard communication program for hazardous chemicals used in the work place.
