29 CFR 1926.502(e)(1): Positioning device systems were not rigged such that an employee could not free fall more than 2 feet (0.9 m):    On or about April 17, 2017, employees were exposed to fall hazards when they installed metal retainer straps from a Genie aerial lift (Model S-45 / Serial Number S4511-16220) between heights of 6 and 10-feet from the concrete surface and the employer provided a body harness with 6-foot lanyard that allowed a 17.5-feet fall;

29 CFR 1926.760(a)(1): Except as provided by paragraph (a)(3) of this section, each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level shall be protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems.    On or about April 18, 2017, employees were exposed to fall hazards when employees installed cable bracing and the employee stood on the metal rafters 15-feet 4-inches from the concrete surface using the employer provided a body harness, 6-foot lanyard, and 6-foot anchorage connector that allowed an 18.5-feet fall.

PERSHING RD., DECATUR, IL 62526.    MAYA ERECTORS LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.20(B)(2), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1124418, CITATION NUMBER 1, ITEM NUMBER 1, AND WAS AFFIRMED AS A FINAL ORDER ON APRIL 13, 2016 WITH RESPECT TO A WORKPLACE LOCATED AT CARROLL & 6TH ST. On or about April 18, 2017, employees were exposed to electrical shock hazard when the outer insulation of the flexible cables was cut along the sharp edges of the sheet metal used to erect the metal building and the inner wiring was exposed on the following:  a) an orange flexible cable that lead from an electrical generator to power a yellow Dewalt Versa-Clutch Screwdriver Model DW368; and  b) an orange flexible cable that lead from an electrical generator to power a black and red Kett metal shear.

On or about April 18, 2017, the employer did not remove from service the following cords and employees were exposed to electrical shock hazard from:   a) an orange flexible cable that lead from an electrical generator to power a yellow Dewalt Versa-Clutch Screwdriver Model DW368 and the outer insulation was torn exposing the inner wiring;  b) an orange flexible cable that lead from an electrical generator to power a black and red Kett metal shear and the outer insulation was torn exposing the inner wiring;  c) a yellow Dewalt Versa-Clutch Screwdriver Model DW368 with outer insulation torn exposing the inner wiring;  d) a Kett metal shear whose black flexible cable with outer insulation torn exposing the inner wiring; and   e) a yellow Dewalt Versa-Clutch Screwdriver Model DW268 with outer insulation torn exposing the inner wiring.
