29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) Line # 3; On or about April 15, 2017 a lock-out / tag-out device was not utilized on the line # 3 day tank to isolate the energy when cleaning the tank.
