On or about April 19, 2017, at the above addressed jobsite, employees performing carpentry operations on the second floor loft were exposed to falls of twelve (12) feet five (5) inches above the ground due to no fall protection and ten (10) feet above the basement floor from the ground floor, due to no guardrails.
