29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a)  1528 W Girard Ave - On or about 4/28/17, employees were accessing the 2nd floor of the structure using an aluminum step ladder that did not extend at least 3 feet about the upper landing area exposing employees to falls of approximately 15 feet.  (b)  1528 W Girard Ave - On or about 4/28/17, employees were accessing the rafters and top caps of the framing using a 6ft fiberglass step ladder, exposing employees to falls of approximately 21 feet.
