29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a- 69 & 71 Bradhurst Avenue, Lakewood NJ: An employee, not using head protection, was exposed to struck by injuries when setting and installing roof trusses while working beneath and next to these trusses as they were being flown in and supported by an overhead crane.  Condition observed on or about 03/12/18.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  a- 69 & 71 Bradhurst Avenue, Lakewood NJ: Employees were exposed to fall hazards when utilizing an A-framed ladder in the closed position, and resting unsecured against a wall, to access and egress the upper walking/working surface of a residential structure. Condition observed on or about 03/08/18.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was an alternative fall protection measure provided:  a- 69 & 71 Bradhurst Avenue, Lakewood NJ: Employees, not using fall protection, were exposed to fall hazards in excess of 10 feet to the ground surface below when setting and installing roof trusses while walking and working from the top plates of a residential structure's exterior walls. Condition observed on or about 03/08/18.   Strong Contractors, Inc. was previously cited a Repeat citation for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1263810, citation number 1, item number 2 and was affirmed as a final order on 10/03/17, with respect to a workplace located at 270 Gordon Road, Robbinsville NJ.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

Condition observed on or about 03/08/18.   Strong Contractor's Inc. was previously cited for a Serious violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(13), which was contained in OSHA inspection number 1248058, citation number 1, item number 1 and was affirmed as a final order on 10/03/17, with respect to a workplace located at 270 Gordon Road, Robbinsville NJ.
