29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a) On or about 05/02/17, at the Pinnacle Lutheran Church, located on 250 Pinnacle Road, in Rochester, New York, employees were installing roofing materials including roof shingles 14 feet above ground level without fall protection. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On or about 05/02/17, at the Pinnacle Lutheran Church, located on 250 Pinnacle Road, in Rochester, New York, a portable ladder in use to gain access to a roof top was not positioned so that the side rails extended 3 feet above the landing.  The roof edge measured 14 feet above ground level.  NO ABATEMENT CERTIFICATION REQUIRED
