1910.1200(h)(3)(ii) The physical, health, simple asphyxiation, combustible dust, and pyrophoric gas hazards, as well as hazards not otherwise classified, of the chemicals in the work area; 29 C.F.R. �1910.1200(h)(3)(ii): The employer did not adequately train employees on potential combustible dust hazards: a) Compounding Area: Where sodium lauryl sulfate, a combustible dust, was transferred to a vessel via a hose and nozzle system, employees were not trained on potential combustible dust hazards associated with that transfer. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.1200(f)(6): The employer did not ensure that each container of hazardous chemical in the workplace was labeled, tagged or marked with the product identifier, signal word, hazard statement(s), pictogram(s), precautionary statement(s) and name, address, and telephone number of the chemical manufacturer, importer, or other responsible party: a) Dental pre-weigh: The employer did not ensure that transfer containers, including, but not limited to spray bottles of 70% isopropyl alcohol, were properly labeled according to the requirements of the 2012 GHS standard. The bottles did not contain the signal word, hazard statement, pictogram, precautionary statement and the contact information for the manufacturer, on or about 7/6/17. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
