29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: Bldg. 2490, Kitchen Area: The pedestal fan (Air King model 9224) power supply cord was missing its ground prong. The cord was plugged into an energized receptacle. The receptacle was not GFCI protected, exposing employees to electrical hazards.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries. Hoppy Shop, Big Trucks Bay Area: A portable fire extinguisher was not mounted and identified. The fire extinguisher was placed on the floor exposing employees to trip and fall hazards. The U.S. AIR FORCE - BEALE was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.157(c)(1), which was contained in OSHA inspection number 905464, citation number 02, item number 001 and was affirmed as a final order on 12/03/2013, with respect to a workplace located at 1951 Edison Drive, Bldg. 1086, Suite 530, Beale Air Force, CA 95903.
