29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) 222 South Market Street Elizabethtown , PA - Employees were observed performing roofing activity on a steep roof 10 in 12 pitch without the benefit of a complete fall protection system, thereby exposing employees to a fall of 33 feet to the ground below, on or about May 5, 2017. No abatement certification or documentation required.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) 222 South Market Street Elizabethtown , PA - Employees were observed using a 40 foot fiberglass extension ladder that did not extend at least 3 feet above the landing, thereby exposing employees to a fall hazard of approximately 33 feet to the ground below, on or about May 8, 2017. No abatement certification or documentation required.
