OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to caught between hazards.  a) On or about ­­­05/04/17, in the Pem area; Bracker Riveter #582007 was operated using a two hand control that was configured in such way that it does not require synchronous use of both hands to initiate the machine cycle. Employees were able to cycle the machine by keeping one button pressed down and actuating the other button later, exposing employees to caught between hazards.     NO ABATEMENT CERTIFICATION REQUIRED
