29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided or used wherever it was necessary by reason of hazards of processes or environment, encountered in a manner capable of causing injury or impairment in the function of any part of the body through physical contact:  a) On or about 08/07/19 in the Welding department; employer required employees to use plant air (unregulated @ 118 psig.) to pressurize a metal container to identify weld leaks and did not provide any barrier or shield to protect employees against being struck by a cover or a bolt should unexpected over pressurization of the container occur.   b) On or about 08/07/19 in the Welding department; employer required employees to use plant air ( unregulated @ 118 psig. ) to pressurize an enclosed metal container to identify weld leaks and only required safety glasses for eye and face protection should an unexpected over pressurization of the container occur resulting in employee exposure to struck by hazards.     ABATEMENT DOCUMENTATION REQUIRED

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees when they were exposed to struck-by hazards during leak testing of a metal container :  a) On or about 08/07/19 in the Welding department; employer wrote a leak testing procedure to identify and repair leaks on an assembly housing (welded metal container 40" x 16.5" x 15" -  4 cubic feet).  Employer provided unregulated plant air (118 psig.) to pressurize the metal container, did not provide with any pressure relief device for the container during the leak testing and provided an oil based liquid for lubrication of bolts used to secure the container covers.  During a leak testing of an assembly housing (welded container), one of the six covers, bolted to the housing to seal the openings, unexpectedly and forcefully detached striking an employee causing serious injury ( severe laceration to arm and abdominal contusions).       ABATEMENT DOCUMENTATION REQUIRED
