29 CFR 1910.1200(h)(3)(iv): The details of the hazard communication program developed by the employer did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:        Maintenance/Housekeeping:  Employees were not trained on the new label elements (e.g., pictograms and signal words) and the new SDS formation by December 2, 2013, as required by the revised Hazard Communication Standard.  Employees handle and use hazardous chemicals such as, spray paints, gasoline, paint thinners, various oils,  and lubricants.

29 CFR 1926.501(a)(2): Employees were allowed to work on surfaces which did not have the requisite strength and structural integrity:    Casino, Tent:  On April 4, 2017, employees were working at a height of approximately 45 feet on top of a tent replacing fans. The tent roof ripped causing an employee to fall through the hole a distance of approximately 4 feet,  a supporting member breaking his fall.  The employer did not ensure the structural integrity of the tent roof.
