This saw included electrical and kinetic energy sources, and the employer did not address the need to allow the blades to stop turning prior to performing maintenance.  An employee experienced the amputation of several fingers when she was clearing a jam from the table saw while in operation.  This condition resulted exposure to amputation hazards.    (b) L&R Pallet Service, Inc, 3855 Lima Street, Denver, CO  80239:  On and before April 18, 2017 the employer did not develop procedures to control hazardous energy for machines with multiple energy sources such as, but not limited to, the two 1 Man Pallet Dismantlers.  These machines included both electrical and hydraulic hazardous energy sources when replacing the blades.  This condition resulted exposure to amputation hazards.
