29 CFR 1910.95(k)(1): The employer did not train each employee who is exposed to noise at or above an 8-hour time-weighted average of 85 decibels in accordance with the requirements of 29 CFR 1910.95(k):          (a)    L& R Pallet Service, Inc. at 3855 Lima Street, Denver CO 80239: On and before 5/30/17, the employer did not provide effective training to all employees who are exposed to noise levels above 85 decibels during their routine work shifts.  Noise monitoring indicated that several employees are exposed to noise hazards in excess of 90dBA with an 8-hour Time Weighted Average.

Abatement Note:  The employer shall ensure that each employee can demonstrate knowledge of all the following:      (1)        Why the respirator is necessary and how improper fit, usage, or maintenance can compromise the protective effect of the respirator;      (2)        What the limitations and capabilities of the respirator are;      (3)        How to use the respirator effectively in emergency situations, including situations in which the respirator malfunctions;      (4)        How to inspect, put on and remove, use, and check the seals of the respirator;      (5)        What the procedures are for maintenance and storage of the respirator;      (6)        How to recognize medical signs and symptoms that may limit or prevent the effective use of respirators; and      (7)        The general requirements of this section.

(a)    L& R Pallet Service, Inc. at 3855 Lima Street, Denver CO 80239: On or before 5/30/17, an employee operating Band Saw 2 was exposed to 15.4 mg/m3 of wood dust as calculated according to an 8hour TWA.  This condition exposed employees to respiratory hazards in the form of wood dust.

The employee assigned to operate Band Saw 2 was exposed to 1.03 times the Permissible Exposure Limit of 15 mg/m3. This condition exposed employees to respiratory hazards from wood dust.
