29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about May 31, 2017, at Lot 126, 6420 Berkdale Way, Suwanee, Georgia:  An employee performing roofing work was exposed to struck-by hazards from falling shingles, nail guns, and other materials of the employees working above him on the roof while he worked on the ground beneath them without wearing a hard hat.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about May 31, 2017, at Lot 126, 6420 Berkdale Way, Suwanee, Georgia:  Four employees performing roofing work were exposed to fall hazards of approximately 10.2-feet to 10.6-feet when they used two aluminum extension ladders that were not extended three feet above the roof landings.

29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about May 31, 2017, at Lot 126, 6420 Berkdale Way, Suwanee, Georgia:  Four employees performing roofing work were exposed to struck-by hazards from wood chips and nails while operating Hitachi Nv45Ab2 nail guns without using eye protection.  Sergio Resendiz was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 948032, citation number 1, item number 1, and was affirmed as a final order on July 29, 2014, with respect to a workplace located at Lot 44, 214 Avery Street, Marietta, Georgia.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about May 31, 2017, at Lot 126, 6420 Berkdale Way, Suwanee, Georgia:  a) Three employees performing roofing work on an approximately 10/12 slope roof were exposed to a fall hazard of approximately 10.2-feet to greater than 25-feet when they did not use fall protection.   b) An employee performing roofing work on an approximately 10/12 slope roof were exposed to a fall hazard of approximately 10.2-feet to greater than 25-feet when he attached a rope grab connector to a pelican hook connected to a d-ring which he attached to the webbing of his harness in the front at his waist.   Sergio Resendiz was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 948032, citation number 2, item number 1, and was affirmed as a final order on September 11, 2014, with respect to a workplace located at Lot 44, 214 Avery Street, Marietta, Georgia.  Sergio Resendiz was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 899910, citation number 1, item number 1, and was affirmed as a final order on October 6, 2010 with respect to a workplace located at Laurel Creek Lot #20, Smyrna, Georgia.
