29 CFR 1926.503(a)(2)(iv): The employer did not ensure that each employee was trained, as necessary by a competent person in the role of each employee in the safety monitoring system where the system was used: Jobsite: Employees were working on two different roof levels greater than sixty (60) feet above the concrete below using a safety monitor system where the employees were not trained in the use of the system and the role of each employee.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. Jobsite: Employees were working near the edge of the roof approximately seventy feet above the ground without the use of fall protection or a safety monitor system exposing them to falls to the concrete below. Corolla Contracting, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1186469, citation number 1, item number 1, and was affirmed as a final order on January 6, 2017, with respect to a workplace located at 8 Prospect Street, Nashua, NH 03060.
