29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control:  a)  On or about May 31, 2017, the employer did not ensure to write down in the energy control procedures that it was a requirement for employees to test the machine to determine and verify that the Thiele Technologies, Inc., Machine #BCSP1(Serial #HS75OSHA103) machine was locked out.   b)  On or about May 31, 2017, the employer did not ensure to write down on the energy control procedures what were the energy sources that the employees were to test to determine that the Thiele Technologies, Inc., Machine #BCSP1(Serial #HS75OSHA103) was locked out properly.

The guarding device shall be in conformity with any appropriate standards therefore, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle:  a)  On or about June 6, 2017,  the employer did not ensure that a guard was installed on the sewing head to prevent employees from contacting the two needles on the machine on the Thiel Technologies, Inc.,  BCSP1 machine (Serial #HS75OSHA103).   b) On or about June 5, 2017, the employer did not ensure that a guard was installed on the sewing head to prevent employees from contacting the two needles on the machine on the Thiel Technologies, Inc., BCS#3 machine (Serial #HS75OSHA101).  c)  On or about May 23, 2017, the employer did not ensure that a guard was installed on the sewing head to prevent employees from contacting the two needles on the machine on the Thiel Technologies, Inc., BCS#4 machine (Serial #HS75OSHA101).  d)  On or about June 6, 2017, the employer partially guarded the web forming area to prevent employees from contacting the in-going nip points behind the guard on the Thiel Technologies, Inc., BCSP1 machine (Serial #HS75OSHA103).   e)  On or about June 6, 2017, the employer partially guarded the web forming area to prevent employees from contacting the in-going nip points behind the guard in the machine on the Thiel Technologies, Inc., BCS#3 machine (Serial #HS75OSHA101).   f)  On or about June 6, 2017, the employer partially guarded the web forming area to prevent employees from contacting the in-going nip points behind the guard in the machine on the Thiel Technologies, Inc., BCS#4 machine (Serial #HS75OSHA101).
