29 CFR 1910.23(c)(11): (11) Portable ladders used to gain access to an upper landing surface have side rails that extend at least 3 feet (0.9 m) above the upper landing surface.    On or about December 19, 2018, employees were exposed to fall hazards using a portable ladder that did not extend 3 feet above the landing surface to access a working platform that was 7 feet above the concrete floor on the Astraflex Press #4.

(i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following:      On or about December 19, 2018, employees were exposed to fall hazards when working from a unguarded platform that was 7 feet above the concrete floor on the Astraflex Press #4.

29 CFR 1910.106(d)(4)(i): "Construction." Inside storage rooms shall be constructed to meet the required fire-resistive rating for their use. Such construction shall comply with the test specifications set forth in Standard Methods of Fire Tests of Building Construction and Materials, NFPA 251-1969. Where an automatic sprinkler system is provided, the system shall be designed and installed in an acceptable manner. Openings to other rooms or buildings shall be provided with noncombustible liquid-tight raised sills or ramps at least 4 inches in height, or the floor in the storage area shall be at least 4 inches below the surrounding floor. Openings shall be provided with approved self-closing fire doors. The room shall be liquid-tight where the walls join the floor. A permissible alternate to the sill or ramp is an open-grated trench inside of the room which drains to a safe location. Where other portions of the building or other properties are exposed, windows shall be protected as set forth in the Standard for Fire Doors and Windows, NFPA No. 80-1968, which is incorporated by reference as specified in Sec. 1910.6, for Class E or F openings. Wood at least 1 inch nominal thickness may be used for shelving, racks, dunnage, scuffboards, floor overlay, and similar installations.    On or about December 19, 2018, ProAmpac dba Coating Excellence International was utilizing the ink room as a storage location for drums of Class II and Class III Flammable liquids. The ink room does not meet the construction requirements of the standard.

29 CFR 1910.106(e)(9)(iii): "Waste and residue." Combustible waste material and residues in a building or unit operating area shall be kept to a minimum, stored in covered metal receptacles and disposed of daily.  On or about December 12, 2018, employees of ProAmpac dba Coating Excellence International were using cleaning towels and Isopropyl Alcohol to clean drums and other press components on the Astraflex Press #4. Employees were then storing the used cleaning towels in an uncovered plastic barrel near the press.

29 CFR 1910.303(g)(1)(ii): Working space required by this standard may not be used for storage. When normally enclosed live parts are exposed for inspection or servicing, the working space, if in a passageway or general open space, shall be suitably guarded.  On or about December 19, 2018, two empty barrels were placed in front of electrical panels "2C" and "2D" located on the outside of the ink room.
