A safety and health program with policy, hazard assessment, and work site rules was not developed and/or implemented at this job site:     A)    Ron Morrow, conducting work at 15293 Sunset Maple Drive, Perrysburg, OH 43551: On or about June 6, 2017, the employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as, but not limited to, falls during job site operations.

Employees did not use fall protection and were exposed to approximately 12 feet fall hazard from the roof to the ground below.

The box was not listed to be used on the electrical extension cord instead of permanent receptacle. The knockout style receptacle box was laying on the ground of the construction site. Employees were exposed to electrical hazards of 120 volts while connecting/disconnecting electrical cord to/from the box.

The cords were located on the construction site and were used to power equipment such as, but not limited to, Rolair air compressor. The insulation of the extension cords was damaged. Employees were exposed to electrical hazards of 120 volts while handling the cords.
