A safety and health program with policy, hazard assessment, and work site rules was not developed and/or implemented at this job site. a) Ron Morrow, conducting work at 15094 Hickory Creek Court in Perrysburg, Ohio; On or about May 14, 2019, the employer did not have a plan or program to identify job site hazards which assist employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as, but not limited to, falls during jobsite operations. Ron Morrow was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.20(b)(1), which was contained in OSHA Inspection Number 1237864, citation number 1, item number 1a and was affirmed as a final order on July 11, 2017, with respect to a workplace located at 15293 Sunset Maple Drive in Perrysburg, Ohio.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) Ron Morrow, conducting work at 15094 Hickory Creek Court in Perrysburg, Ohio; On or about May 14, 2019, the employer did not ensure that employees had been adequately trained in the appropriate use of fall protection such as, but not limited to, the nature of fall hazards, the use of personal fall arrest systems, or alternative measures of fall protection. Employees were exposed to approximately 10 feet fall hazards from the roof to the ground below during the sheeting of a 6/12 single story residential roof. Ron Morrow was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.503(a)(1), which was contained in OSHA Inspection Number 1237864, citation number 1, item number 1b and was affirmed as a final order on July 11, 2017, with respect to a workplace located at 15293 Sunset Maple Drive in Perrysburg, Ohio.

Employees were exposed to approximately 10 feet fall hazards from the roof to the ground below.
