29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:  a. On or about June 10, 2017, at the west and south side of the residential building, four employees using pneumatic nail guns to install plywood and fascia on a 6:12 pitch roof were not wearing eye protection, exposing themselves to flying debris and struck-by hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  a.  At the job site, employees working at elevations of 6 feet or more above ground are not protected from fall hazards by the use of a personal fall arrest system.  The employer fails to assure effective protection against falls as required by the standard, as demonstrated on June 10, 2017, when employees doing roof sheathing work on a residential home at 13000 Pechora Ct., Jacksonville, Florida 32246 without fall protection were exposed to fall hazards of 24 and 11 feet from the leading edge of the building.  Malagon Framing, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1095495, citation 02, item 01, issued on October 7, 2015, with respect to a workplace at ICI Homes Lot 86, Jacksonville Beach, 32240.  The final order date was October 21, 2015.    Malagon Framing, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1085487, citation 01, item 01, issued on October 7, 2015, with respect to a workplace at 130 Sandy Beach Lane, Atlantic Beach, 32233.  The final order date was October 21, 2015.    Malagon Framing, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 997772, citation 01, item 01, issued on October 7, 2014, with respect to a workplace at 724 W. Kings College Dr., Saint Johns, 32259.  The final order date was December 1, 2014.
