29 CFR 1926.102(a)(1): Eye and/or face protective equipment was not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: On or about June 06,2017 during the construction of a single family home located at sub-lot 120, Martins Run Dr. in Lorain, OH, employees were observed using pneumatic nail guns to secure structural framing without safety eye-wear or facial protection.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: On or about June 06, 2017, at sub-lot 120 located in Lorain, OH, an employee in the process of completing residential construction activities including; but not limited framing, was exposed to a fall hazard from a height of approximately 10 feet.
