29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.   On or about December 4, 2017, employees were exposed to a fall of approximately 13 feet.  The employees were performing framing activities on a new residential home located at 1264 Sandy Ridge Rd., North Ridgeville, Ohio 44035.  An employee was using an A-frame ladder while working in front of an unguarded wall opening on the second floor.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed   On or about December 4, 2017, employees were exposed to a fall of approximately 13 feet.  The employees were performing framing activities on a new residential home located at 1264 Sandy Ridge Rd., North Ridgeville, Ohio 44035.  An employee was using an A-frame ladder that was leaned against the wall on the second floor.

29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about December 4, 2017, employees were exposed being struck by flying objects.  The employees were performing framing activities on a new residential home located at 1264 Sandy Ridge Rd., North Ridgeville, Ohio 44035.  The employees were using air powered nail guns without wearing safety glasses.  Creekside Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.102(a)(1), which was contained in OSHA inspection number 1239233 citation number 1, item number 1 and was affirmed as a final order on 8/1/2017, with respect to a workplace located at S/L 120 Martins Run drive, Lorain, Ohio 44053.

On or about December 4, 2017, employees were exposed to a fall of approximately 21 feet.  The employees were performing framing activities on a new residential home located at 1264 Sandy Ridge Rd., North Ridgeville, Ohio 44035.  Two employees were working on the roof without using any form of fall protection.   Creekside Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(13), which was contained in OSHA inspection number 1239233 citation number 1, item number 2 and was affirmed as a final order on 8/1/2017, with respect to a workplace located at S/L 120 Martins Run drive, Lorain, Ohio 44053.
