Creekside Construction on or about July 31, 2019, at the address of 271 Kensington Way, Elyria, OH: The employer did not ensure the side rails of the aluminum extension ladder were extended at least three feet above the landing surface. The ladder was extended roughly 2 feet above the eave of the roof.

On or about July 31, 2019, at the address of 271 Kensington Way, Elyria, OH: Employees were exposed to being struck by flying objects. The employees were performing sheeting activities on a new residential home. The employees were using air powered nail guns without the benefit of wearing safety glasses. Creekside Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.102(a)(1), which was contained in OSHA inspection number 1281367 citation number 2 item number 1 and was affirmed as a final order on 01/17/18, with respect to a workplace located at 1264 Sandy Ridge Rd., North Ridgeville, Ohio 44035.

On or about July 31, 2019, at the address of 271 Kensington Way, Elyria, OH: Employees were exposed to a fall of approximately 10 feet. The employees were performing sheeting activities on a new residential home. Two employees were working on the roof without the benefit of any type of fall protection. Creekside Construction LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1281367 citation number 2 item 2 and was affirmed as a final order on 01/17/18, with respect to a workplace located at 1264 Sandy Ridge, North Ridgeville, OH, 44035.
