Creekside Construction on or about July 31, 2019, at the address of 271 Kensington Way, Elyria, OH: The employer did not have a plan or program to identify job site hazards which assist employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards, such as falls during roofing operations, decreasing the probability of employee injury and/or illness.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    Creekside Construction on or about July 31, 2019, at the address of 271 Kensington Way, Elyria, OH: The employer did not ensure the side rails of the aluminum extension ladder were extended at least three feet above the landing surface. The ladder was extended roughly 2 feet above the eave of the roof.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about July 31, 2019, at the address of 271 Kensington Way, Elyria, OH: Employees were exposed to being struck by flying objects. The employees were performing sheeting activities on a new residential home. The employees were using air powered nail guns without the benefit of wearing safety glasses.   Creekside Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.102(a)(1), which was contained in OSHA inspection number 1281367 citation number 2 item number 1 and was affirmed as a final order on 01/17/18, with respect to a workplace located at 1264 Sandy Ridge Rd., North Ridgeville, Ohio 44035.

On or about July 31, 2019, at the address of 271 Kensington Way, Elyria, OH: Employees were exposed to a fall of approximately 10 feet. The employees were performing sheeting activities on a new residential home. Two employees were working on the roof without the benefit of any type of fall protection.   Creekside Construction LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1281367 citation number 2 item 2 and was affirmed as a final order on 01/17/18, with respect to a workplace located at 1264 Sandy Ridge, North Ridgeville, OH, 44035.
