29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on flat roofs with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, warning line system, or a safety monitoring system:  a) Jobsite: Two story corner building, roof - Employees working within 1 foot of unprotected roof sides and edges, while conducting a roof removal operation, were exposed to fall hazards of approximately 25 feet without the use of a fall protection system.  b) Jobsite: Two and 1 half story building adjoined to the corner building, roof - Employees working within 1 foot of unprotected roof sides and edges, while conducting a roof removal operation, were exposed to fall hazards of approximately 30 feet without the use of a fall protection system. 29 CFR 1926.850(a): Prior to permitting employees to start demolition operations, an engineering survey was not made by a competent person of the structure to determine the condition of the framing, floors, and walls, and possibility of unplanned collapse of any portion of the structure, including adjacent structures where employees may be exposed. The employer did not have in writing evidence that such a survey was performed.  a) Jobsite: An engineering survey was not completed by a competent person prior to the employer permitting his employees to conduct demolition operations on the 2 story building.   Noted on or about 5/23/17.

29 CFR 1926.859(g): During demolition, continuing inspections by a competent person were not made as the work progressed to detect hazards resulting from weakened or deteriorated floors, or walls, or loosened material. Employees were also permitted to work where such hazards existed without being corrected by shoring, bracing, or other effective means:  a) Jobsite: The employer did not have a competent person conduct continuing inspections as the work progressed to detect hazards associated with demolition operations such as structural building collapses.  A large wooden exterior panel fell from the top of the building and landed on a worker beneath it.  The worker suffered head and back injuries.  Noted on or about 5/23/17.
