29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or  were not packed or stored to prevent deformation of the facepiece and exhalation valve  Employees working in the warehouse were exposed to respiratory hazards in that the voluntary use of N95 filtering face piece respirators were not stored to prevent contamination and deformation.

29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks was not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in Sec. 1910.6:    Employees working in the warehouse were exposed fire and explosion hazards in that a compressed gas cylinder containing acetylene was stored near engine oil in an area that did not have a sign communicating the type of gas that was to be stored there.  The U.S. Department Of The Interior, Bureau Of Land Management was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.101(b), which was contained in OSHA inspection number 1155473, citation number 2, item number 1 and was affirmed as a final order on 2/13/2017, with respect to a workplace located at 5665 Morgan Hill Road, Carson City, NV 89701.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:  Employees working in the warehouse were exposed to respiratory hazards in that they were not provided a copy of Appendix D of the respirator standard prior to voluntary use of N95 filtering face piece respirators , and were unaware of the limitations and hazards of respirator use.  The U.S. Department Of The Interior, Bureau Of Land Management was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(c)(2)(i), which was contained in OSHA inspection number 1051269, citation number 1, item number 1 and was affirmed as a final order on 5/11/2015, with respect to a workplace located at South Fork Fire Station 30147 Hwy 178 Onyx, CA 93255.
