29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: a) Main Panel - The employer did not ensure that the space in front of the main electrical panel located in the pallet break down area permitted ready and safe access to the panel, in that access to the panels was hindered by wood and extraneous material, thereby exposing employees to possible injuries, on or about June 20, 2017. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

This condition exposes the inner energized conductors to damage. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date, and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasions: a) Notcher Machine: On or about June 20, 2017, inner energized conductors were pulled out from the gland nut where they enter Baldor DC drive box for the Roach conveyor. This condition exposes the inner energized conductors to abrasion damage. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date, and method of abatement, and states employees and their representatives were informed of this abatement.
