An employee operating the band saw was exposed to 97.4 dBA.  b) Facility:    On or about February 8, 2018, the employer did not develop an effective hearing conservation program for employees exposed to noise levels about 85 decibels on the A weighted scale (dBA) for an eight hour time weighted average.  An employee building pallets  was exposed to 94.3 dBA.  c) Facility:    On or about February 8, 2018, the employer did not develop an effective hearing conservation program for employees exposed to noise levels about 85 decibels on the A weighted scale (dBA) for an eight hour time weighted average.  An employee helping the band saw operator was exposed to 92 dBA.  d) Facility:    On or about February 8, 2018, the employer did not develop an effective hearing conservation program for employees exposed to noise levels about 85 decibels on the A weighted scale (dBA) for an eight hour time weighted average.  An employee performing pallet repair was exposed to 89.9 dBA.  SK Pallet Inc., was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1244954, citation number 1, item number 2a and was affirmed as a final order on December 1, 2017, with respect to a workplace located at 1000 Loucks Mill Road York PA .  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
