29 CFR 1910.22(a)(1):  All places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition.  SK Pallet Inc. - The employer did not ensure that the overhead building structures and block wall were free from fugitive wood dust accumulation, on or about July 6, 2017.  In the Alternative   29 CFR 1910.141(a)(3)(i): All places of employment shall be kept clean to the extent that the nature of the work allows:  SK Pallet Inc. - The employer did not ensure that the overhead building structures and block wall were free from fugitive wood dust accumulation, on or about July 6, 2017.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.176(c): Storage areas were not kept free from accumulation of materials that constituted hazards from tripping, fire, explosion or pest harborage:  a). SK Pallet, Inc. - Excessive combustible wood dust was on the pallets in the storage area where the trim saw, chop saw, and notcher machine were being used to manufacture the wooden pallets, on or about July 6, 2017.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:   a). SK Pallet, Inc. - On or about July 6, 2017, an employee working on the builder table was exposed to noise at 99.4dBA for 418 minutes while building pallets. The standard is 85dBA for an 8-hour day.  b). SK Pallet, Inc. - On or about July 6, 2017, an employee using the Hitachi chop saw was exposed to noise at 91.8dBA for 419 minutes while cutting wood material. The standard is 85dBA for an 8-hour day.   c). SK Pallet, Inc. - On or about July 6, 2017, an employee using a trim saw was exposed to noise at 92.4dBA for 430 minutes while trimming wood materials. The standard is 85dBA for an 8-hour day.   d). SK Pallet, Inc. - On or about July 6, 2017, an employee working at the repair table was exposed to noise at 92.5dBA for 413 minutes while repairing pallets. The standard is 85dBA for an 8-hour day.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
