29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces were kept in a clean, orderly, and sanitary condition. Employees working by the combination sheer break in the metal shop were exposed to fire and smoke inhalation hazards in that the floor had not been swept, resulting in a significant build up of flammable and potentially combustible dust one inch in depth.
