29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws. Building 991, Naval Facilities Metal Shop: On or before April 27, 2021 sheet metal employees and welders used extension cord(s) to power portable equipment for metal fabrication. The extension cord plug was pulled away from the insulation exposing the inner insulated wires. Employees were exposed to electrical shock hazard(s).

Except as provided in paragraphs (f)(7) and (f)(8) of this section, the employer shall ensure that each container of hazardous chemicals in the workplace is labeled, tagged or marked with either: Building 991, Naval Facilities Metal Shop: On or before April 27, 2021 sheet metal employees and welders had used unlabeled containers of cutting oil lubricant for the cutting processes conducted at the metal shop using band saws, and other cutting hand tools. Employees were exposed to unlabeled containers containing a chemical product such as cutting oil lubricants which can result in acute physical reactions.
