29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. On or about 21 June 2017 and at times prior thereto, a worker was performing roofing activities and handing materials from an upper level to a worker on a lower level who was not wearing head protection, exposing the worker to the hazard of being struck in the head by falling tools and material.

29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. On or about 21 June 2017 and at times prior thereto, two roofers without eye protection were operating pneumatic nail guns on the roof of a residence under construction, exposing the roofers to the hazard of being struck by flying materials and debris.

On or about 21 June 2017 and at times prior thereto, two roofers were performing roofing activities on a residence under construction without fall protection, exposing the roofers to the hazard of falling approximately 15 feet to the ground below.
