29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. a) BP-Husky Refining, LLC located in Oregon, Ohio: On or about May 24, 2017, the employer did not ensure an employee performing cleaning operations on the strainer basket for the benzene stripper unit 2, stripper bottoms pump, utilized procedures to locked out and deenergized the piping leading to the strainer basket prior to performing cleaning operations.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program were understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls were acquired by employees. a) BP-Husky Refining, LLC located in Oregon, Ohio: On or about May 24, 2017, the employer did not ensure an employee performing cleaning operations on the strainer basket for the benzene stripper unit 2, stripper bottoms pump, was provided with adequate training on the lockout standard. The employee did not recognize the potential for stored pressure in the line and the need to ensure the line was locked out and deenergized prior to performing cleaning operations on the strainer basket.
