29 CFR  1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment:   BP-Husky Refining, LLC - Oregon, Ohio:  On or about December 7, 2021, the employer did not ensure that a PPE hazard assessment had been conducted for the task of changing out methanol totes in the LPG Unit.  Failure to  perform and certify the PPE assessment can lead to the lack of use of necessary PPE when needed, and potential exposure to chemical hazards, such as methanol.

29 CFR  1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes:  BP-Husky Refining, LLC - Oregon, Ohio:  On or about December 7, 2022, the employer did not ensure that employees tasked with changing out the methanol totes in the LPG Unit were provided with, and required to wear chemical protective gloves.  Employees were exposed to dermal contact hazards, such as dermatitis, dry/cracking skin, and numbness, from the methanol released during the change-out of the portable totes.
