Building Systems, Inc., 2000 West Grandview Boulevard, Erie, PA 16509, second floor, on or about June 28, 2017 and times prior thereto: Employees were walking/working in proximity to open floor holes that were 5-inches and 2-inches in diameter. The floor holes were not protected with covers. A.) Building Systems, Inc., 2000 West Grandview Boulevard, Erie, PA 16509, second floor, on or about June 28, 2017 and times prior thereto: Employees were walking/working in proximity to an open floor hole that was seven to eight inches in width around the vertical duct work. The floor hole were not protected with covers.
