29 CFR 1910.179(n)(3)(xi): When starting the bridge and when the load or hook approaches near or over personnel, the warning signal shall be sounded:    Northeast, saw department, single shipping and receiving dock, Demag 10 Ton serial no. 95433 overhead bridge crane: on or about June 21, 2017, installed horn was not sounded when starting the bridge to provide advance warning for personnel of the approaching load in the travel path. Employees using remote controller and other working in the area were exposed to swinging load struck-by hazards during regular load handling tasks including unloading steel bundles.    Abatement documentation is required for this item in accordance with the requirement of 29 CFR 1903.19(d).

29 CFR 1910.184(c)(9): All employees shall be kept clear of loads about to be lifted and of suspended loads:    Northeast, saw department, single shipping and receiving dock, Demag 10 Ton serial no. 95433 overhead bridge crane: On or about June 21, 2017, employees were permitted to operate a crane for regular load handling tasks including unloading steel bundles while hand-guiding suspended loads to a temporary storage location. Employees were not required to keep clear of loads about to be lifted and from suspended loads. Employee did not keep clear of the load and was exposed to being struck by swinging loads against stationary objects.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Northeast, saw department, single shipping and receiving dock, Demag 10 Ton serial no. 95433 overhead bridge crane, type VI sling 8-foot long nylon return/reverse eye: on or about June 21, 2017, employees hand-guided basket-hitched suspended loads to a temporary staging location. Employees were exposed to crushing hazards from sling failure where the provided sling was not additionally supplied with padding and/or protective equipment/material to prevent snags, punctures, tears, cuts, and broken or worn stitches.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.184(d): Inspections. Each day before being used, the sling and all fastenings and attachments shall be inspected for damage or defects by a competent person designated by the employer. Additional inspections shall be performed during sling use, where service conditions warrant. Damaged or defective slings shall be immediately removed from service:  Northeast, saw department, single shipping and receiving dock, Demag 10 Ton serial no. 95433 overhead bridge crane, type VI sling 8-foot long nylon return/reverse eye: on or about June 21, 2017, employees hand-guiding suspended loads using a basket-hitched sling were exposed to crushing hazards from sling failure when it was not adequately inspected by a competent person able to detect damages and remove it from service due to snags, punctures, tears, cuts, and broken or worn stitches.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
