A tarp was attached to the basket increasing the surface area of the aerial lift basket.

29 CFR 1926.103: The requirements applicable to construction work under this section are identical to those set forth at 29 CFR 1910.134 of this chapter. 29 CFR 1910.134(f)(2): Employee(s) using a tight-fitting facepiece respirator were not annually fit tested: (Construction Reference 1926.103) (a) On June 29, 2017, employee(s) of Oak Roofing, Inc. who were required to use 3M 6800 full-face respirators with P100 filters were not annually fit tested to ensure a proper fit. Oak Roofing, Inc. was previously cited for this OSHA Safety and Health Standard 1910.134(f)(23) which was contained in OSHA inspection number 1065361, Citation number 1, Item number 2b with respect to the workplace located at 1033 Ontario St., Oak Park, IL 60302, and was affirmed as a final order on September 2, 2015.
