29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii): On or about July 12, 2017, employees were exposed to a 17.2 foot fall hazard while engaged in masonry work from a metal frame scaffold that was not fully planked and the employer did not ensure the scaffold was fully planked on all working levels. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: On or about July 12, 2016, employees were exposed to a 17.2 foot fall hazard while engaged in masonry work from a metal frame scaffold and were not protected from a fall while. The employer did not ensure employees were adequately protected from a fall while performing work activities. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
