29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact: (a) Roof over the Side Porch - Two employees worked together using a grinder and a leaf blower in the process of installing copper flashing on the roof and did not use any eye protection, exposing them to the hazard of getting struck in the eyes by flying particles, on or about July 12, 2017. Corrected during the inspection.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system; or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a safety monitoring system: (a) Roof over the Side Porch - Two employees worked installing copper flashing on the roof and did not use any fall protection, exposing them to the hazard of falling 10 feet to the ground below, on or about July 12, 2017. Corrected during the inspection.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) Roof over the Side Porch - Two employees used a portable extension ladder to gain access to the roof to install copper flashing. This ladder did not extend three feet above the roof (upper landing) surface, exposing the employees to the hazard of falling 10 feet to the ground below, on or about July 12, 2017. Corrected during the inspection.
