29 CFR 1926.1053(b)(22): Employees using ladders were carrying objects or loads that could cause the employee to lose balance and fall:  a) 1804 Delaware Avenue, Wilmington DE - On or about October 6, 2017, the employer failed to prevent employees from carrying objects or loads that could cause the employees to lose balance and fall.  While carrying a 5-gallon pail of Bulldog Modified Bitumen Adhesive in one hand, an employee descended an extension ladder from the roof of a residential building.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:  a) 1804 Delaware Avenue, Wilmington, DE - On or about October 6, 2017, the employer failed to provide employees with effective training on the proper use of ladders.  An employee used an extension ladder to descend from the roof of a residential structure while carrying materials which could cause the employee to lose their balance and fall.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a) 1804 Delaware Avenue, Wilmington, DE - On or about October 6, 2017, the employer failed to prepare a written certification of training provided to employees related to the recognition of fall hazards and the methods and procedures to be used to minimize the hazards.  Abatement Note: The training program must enable the employees to recognize the hazards of falling and must train each employee in the procedures to be followed in order to minimize these hazards.  Each employee must be trained by a competent person in the following:  1. The nature of fall hazards in the work area; 2. The correct procedures for erecting, maintaining, disassembling, and inspecting the fall protection systems to be used; 3. The use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used; 4. The role of each employee in the safety monitoring system when this system is used; 5. The limitations on the use of mechanical equipment during the performance of roofing work on low-sloped roofs; 6. The correct procedures for the handling and storage of equipment and materials and the erection of overhead protection; 7. The role of employees in fall protection plans; 8. The standards contained in this subpart.  The employer must verify compliance with paragraph (a) of this section by preparing a written certification record that will contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.
