29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:     a) Accuride jobsite, 1015 E. 12th St. in Erie, PA, north side of Building 12.  On or about July 12, 2017, employees performing roof-work were not protected from falling 16 feet to the ground.  Employees were wearing personal fall protection harnesses but were not attached to the ropes and anchorage points.     Leon Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1143781 citation number 1, item number 2 and was affirmed as a final order on October 12, 2016, with respect to a workplace located at 1712 W. 38th St. in Erie, PA 16508.    Leon Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1169940 citation number 1, item number 2 and was affirmed as a final order on October 12, 2016, with respect to a workplace located at 613 W. 29th St. in Erie, PA 16508.
