29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program was understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls were acquired by employees:     On or about July, 18, 2017, the employer lockout/tagout training was inadequate, in that, it did not ensure affected employees understood the purpose and function of their energy control program. Affected employees were performing cleaning work inside of a case elevator after a machine guard was removed and the machine was not locked out.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:  On or about July 18, 2017, the number 108 line case elevator machine was not locked out during cleaning. The barrier guard for access was removed and an employee placed their upper body into the base area of the machine. The internal vertical conveyor system lifts cartons of lubrication cans to the upper conveyor system. The case elevator was started up while the employee was cleaning the inside of the machine.

29 CFR 1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible:     On July 18, 2017, on the #108 line case elevator there was barrier guarding that was not adequately secured. There was a guard at the side of the machine and guarding at the horizontal feed conveyor to the case elevator that were attached with wing nuts that are easily removed by hand.
