29 CFR 1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   On or about January 21, 2025, the employer did not ensure the developed and documented hazardous energy control (lockout/tagout) procedure was utilized for the control of the hazardous electric and pneumatic energy sources when machine operator employees were engaged in service and maintenance activities, such as unjamming the 804 Line Palletizer Machine.  Machine operators reach into and/or enter into the machine to unjam product cases (boxes) without utilizing the developed and documented hazardous energy control procedure, exposing them to the unexpected startup of the palletizer machine.      Malco Products, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(4)(i) which was contained in OSHA inspection number 1681198, citation number 1, item number 1-a, and was affirmed as a final order on 9/6/2023, with respect to a workplace located at 12155 Fisher Avenue, NE, Alliance, Ohio.

On or about January 21, 2025, the employer did not ensure a machine operator working on the 804 Line was provided training to ensure the purpose and function of the employer's energy control (lockout/tagout) program was understood and the employee acquired the skills required for the safe application, usage, and removal of energy controls.  804 Line palletizer machine operators reach into and/or enter into the 804 Line palletizer machine to unjam product cases (boxes) without receiving training in the recognition of applicable hazardous energy source, types and magnitude of the energy available in the workplace, methods and means necessary for energy isolation and control (per 1910.147(c)(7)(i)(A), or being instructed in the purpose and use of the energy control procedures [per 1910.147(c)(7)(i)(B).    Malco Products, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(4)(i)(A) which was contained in OSHA inspection number 1681198, citation number 1, item number 1-b, and was affirmed as a final order on 9/6/2023, with respect to a workplace located at 12155 Fisher Avenue, NE, Alliance, Ohio.
