On or about July 5, 2023, the employer did not ensure the 804 Line, Pearson Packaging/Carton Erector Machine operator(s), utilized the developed and documented energy control procedures to control the potentially hazardous energy sources (electrical, pneumatic, vacuum) of the machine tasked with unjamming boxes/cartons inside the machine.

29 CFR 1910.147(c)(7)(i)(A):Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. On or about July 5, 2023, the employer failed to ensure machine operator(s) performing service and maintenance activities on the 804 Line, Pearson Packaging/Carton Erector Machine received training in the methods and means necessary for energy isolation and control. Employees operating the 804 Line, Pearson Packaging/Carton Erector Machine engaged in the service and maintenance activities on the machine when clearing jammed cardboard boxes/cartons from inside the machine.

29 CFR 1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a. On or about July 5, 2023, the employer failed to ensure operators of the 804 Line, Pearson Packaging/Carton Erector Machine were protected by one or more methods of machine guarding from hazards created by the point(s) of operation of the movement of the suction cup arm and packaging folding arm, and the ingoing nip points and rotating parts of the machine's chains and sprockets. b. On or about July 5, 2023, the employer failed to ensure operators of the 808 Compounding Line were protected by one or more methods of machine guarding from hazards created by the ingoing nip points and rotating parts of compounding line's two bucket conveyors.
