29 CFR 1910.37(a)(2): Exit routes must be arranged so that employees will not have to travel toward a high hazard area, unless the path of travel is effectively shielded from the high hazard area by suitable partitions or other physical barriers:  The exit route from the rear door contained five 12 inch diameter square holes approximately 1 to five inches in depth.  The path of travel was not shielded from the holes.

29 CFR 1910.157(g)(1): Where the employer has provided portable fire extinguishers for employee use in the workplace, the employer shall also provide an educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting:  The Emergency Preparedness and Response Plan instructs upon discovery of a fire, if it cannot be easily extinguished to activate the fire alarm and evacuate the building.  All staff members were not provide an educational program to familiarize them with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting.

The Dept. of Agriculture-FSA was previously cited for a violation of this occupational safety and health standard or similar standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1216667 Notice number 1, Item 1 and was affirmed as a final order on 5/31/2017 with respect to a workplace located at 1848 Kountry Lane STE 2, Fort Dodge, IA  50501.    The Dept. of Agriculture-FSA was previously cited for a violation of this occupational safety and health standard or similar standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1167409 Notice number 1, Item 4 and was affirmed as a final order on 9/28/2016 with respect to a workplace located at 1513 N. Ankeny, Blvd. Suite 1. Ankeny, IA 50021.                                          Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos, which you believe will be helpful.  The abatement certification sheet is enclosed with the notice(s).
