29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) Top of roof - On or about July 20, 2017 employees working of roof applying plywood decking 12 feet high were not protected from falling off with a fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) Front and rear of building - On or about July 20, 2017 employees applying decking to the roof gained access to the roof using aluminum extension ladders that did not extend 3 feet above the roofs edge.
